Manual removal is using special tools to remove the wax from the outer part of the ear canal. Sometimes it just helps to physically lift the piece of wax if it is quite solid and deep. This is because with water syringing the piece of solid wax could be stopping the water from getting behind it and washing it out. On the other hand with microsuction, quite often we will refer to putting in more oil, however in some instances, the wax is so dry it can come away from the wall of the ear canal itself and be gently pulled out with forceps.
